To participate in the 2022 Championships, teams must:
1. Complete and submit this form by FRIDAY, DECEMBER 17, 2021 at 11:59:59pm.
2. Remit membership dues of $120 by personal, bank, or school check, or wire transfer.
DEADLINE: FRIDAY, DECEMBER 17, 2021 (invoice will be emailed to contacts)
3. Remit entry fee in the amount of $500 (1 gender) or $1000 (both genders) payable by personal, bank, or school check,
DEADLINE: SATURDAY, FEBRUARY 11, 2022 (invoice will be emailed to contacts)
